For many Weston County residents, quality health care does not stop at the hospital doors. Through its Home Health and In-Home Services programs, Weston County Health Services delivers skilled medical care, personal support, and everyday assistance directly to patients where they are most comfortable — at home.

Established in 1993, WCHS Home Health and In-Home Services bring more than 25 years of experience helping patients maintain independence, safety, and quality of life. The programs are guided by a simple but critical goal: provide compassionate, reliable, high-quality care while respecting each patient’s dignity and individual needs. That mission translates into a strong presence across the county.

Home Health: Skilled Care With a Personal Touch

WCHS Home Health provides physician-directed medical services for patients recovering from illness or surgery, or managing chronic conditions. Services include skilled nursing, physical, occupational, and speech-language therapy, certified nursing assistants, medication management, and coordination of care.

So far this year, Home Health staff have completed 3,054 patient visits, reflecting strong demand and trust from local residents. Visits include 1,273 nursing visits, 1,183 home health aide visits, 417 physical therapy visits, 91 occupational therapy visits, and 49 speech-language therapy visits.

To reach patients throughout Weston County, staff have driven more than 20,590 miles year to date, ensuring continuity of care across a wide geographic area.

“Our focus is helping patients maintain independence and quality of life in their own homes,” said Heather Boyer, RN, Director of Home Health. “That means providing skilled medical care in a compassionate, reliable, and personalized way.”

Patients frequently cite consistency and professionalism as program strengths, helping reduce hospital readmissions and support recovery in familiar surroundings.

In-Home Services: Supporting Daily Living

While Home Health addresses medical needs, WCHS In-Home Services helps residents manage daily tasks that may become challenging due to age, illness, or disability, allowing individuals to remain safely in their homes.

Since January 1, In-Home Services has assisted 62 recipients, delivering 2,944 hours of care. Services include homemaker support, personal care, chore services, and in-home respite care.

Staff also provide extensive case management, completing 629 follow-up contacts, 164 quarterly visits, 51 initial evaluations, and 46 renewal evaluations this year. In-Home Services staff have driven an estimated 11,405 miles to reach clients across the county.

“In-home services are about more than tasks,” said Seth Parsons, ACC, In-Home Services Manager. “They’re about preserving dignity, helping people stay independent, and giving families peace of mind.”

A Community-Based Approach to Care

Both programs share core values including integrity, quality, respect, fiscal responsibility, and skilled, caring people. Care begins with a referral, and plans are developed collaboratively with physicians, patients, and families, adapting as needs change.
